- Name and contact information (email and phone number) to request materials or schedule a research visit.
- Please indicate service being requested (media duplication, photocopying, or scanning).
- What materials you are requesting: (e.g., Call Number or Collection Name and Number, Box Number, Folders—example, MG 24 Box 1 Folders 5–6)
- Duplication Costs: $1 per image for scanning; $1 per page for photocopies (limit of 100 pages) plus $5 for postage and handling; and $20 for media duplication (audio cassette tape or VHS videotape)—limit five items per request.
- Limitations or restrictions may apply due to copyright or the condition of requested documents, media, and publications. The Ä¢¹½tv Special Collections and University Archives reserves the right to decline duplication services.
